I would however prefer that the scope of 'replace-all' be a persistent radio-button, and that there be only one button for replace-all -- that seems to be the way most software does it.
No thanks. I'm constantly shifting between replace all in doc and replace in selection. I prefer having separate buttons, and it annoys me to have to unhide the buttons constantly (i.e. whenever I start Geany again).
