Thats indeed set by the GTK theme, here the box gets a bright border (dark theme here) thats fairly visible and the vertical bar cursor flashes (for a while anyway, then for some reason it stops) but thats GTK3 on Linux. I'm not sure what capability GTK2 themes have.

The Scintilla block cursor was contributed by someone who wants it for their app, its not supported or loved by the Scintilla maintainer, so making it do anything more than it currently does is unlikely to happen.
