OK, the replacement is only made on the first save.

Also, I just fixed a bug where if the extension is not the default one,
the untitled.foo string is not replaced. Perhaps that was the problem?

Yup, just found that was my problem, I'd used a C++ .hpp header & it failed but if I used a C++ .cpp header it worked because it matched the standard extension.
